Shipping
Shipping is currently set at a flat rate of $9.00 for ground service, $17.00 for 2nd day air and $25.00 for Next Day Air. (This does not include orders shipped outside the continental United States.)

Pricing
Prices are subject to change without notice. The invoice will reflect pricing in effect at the time of shipment. Quoted prices do not include sales tax. Sales tax within the state of Utah is 6.5%.

Payment
Extreme Wellness Clinic accepts Visa or Mastercard. No C.O.D. orders.

Orders may be placed by calling 801-335-0529 or toll-free at 888-289-1697. Visa or Mastercard are accepted and can be processed over the phone.

Return Goods Policy
All returns are subject to a 25% restocking fee. All returns must be authorized prior to return. Customer must pay shipping charges when returning product. All returned product must be un-opened and in original packaging. In the event that there is a manufacturer's defect or other situation that would warrant a product return, Extreme Wellness Clinic will issue full credit to the customer, including freight.

Claims
All claims for shortages or errors must be made immediately upon receipt of order to Extreme Wellness Clinic by phone at 888-289-1697. Items held mor than (30) thirty days from date of receipt will not be accepted for credit.
